Situation
A gentleman receiving home care was becoming increasingly isolated after the death of his wife. Although his care needs were being met, staff noticed a decline in his emotional wellbeing, confidence, and interest in daily activities.
Task
While there was no requirement for additional social support within his care package, Emma recognised that his wellbeing was deteriorating and was determined to help him regain a sense of purpose and connection.
Action
Emma took the initiative to learn more about his interests and discovered that he had been an avid gardener and community volunteer for many years. In her own time, she researched local community groups and worked with colleagues, family members, and local organisations to identify suitable opportunities. She arranged introductions, supported him through the anxiety of attending for the first time, and regularly checked on his progress. Emma also encouraged other team members to reinforce his confidence and celebrate his achievements.
Result
Within a few months, he had joined a local gardening project, developed new friendships, and regained confidence and independence. His family reported a significant improvement in his mood and outlook on life. Emma's actions went far beyond the delivery of care tasks; she recognised a need that others may have overlooked and took proactive steps to improve someone's quality of life in a lasting and meaningful way.
